Overview Full time day / Remote (Gary, IN) reputed company’s Congregational Care Network (CCN) is an innovative program partnering with congregations in Gary, Indiana/Allen County to reputed company ongoing emotional and spiritual support to socially isolated or lonely older adults with chronic illnesses. The reputed company worker will play a pivotal role as a liaison with faith-based partners and the reputed company CCN team, collaborating with interdisciplinary teams to deliver personalized care, support patients and families, and connect them to necessary community resources. Effectively communicates and collaborates with reputed company members of the health care team to facilitate the episode of care so the patient receives services in a safe and timely manner, and the patient and family are provided reputed company necessary resources and education.
