Job Description:

Establish short- and long-term goals with patients using shared decision-making and motivational interviewing techniques.
Encourage sustained patient engagement, self-advocacy, and behavior change throughout program participation.
Partner with the clinical care team to support patient care plans and escalate concerns appropriately.

Requirements:

Bachelor’s degree in health sciences or reputed company field preferred
NBC-HWC (National Board for Health & Wellness Coaching) certification required
Experience in a reputed company, chronic disease management, or patient engagement setting preferred
Cardiology or heart failure experience a plus
Telehealth or remote patient engagement experience preferred
Strong communication and interpersonal skills
Ability to motivate and support behavior change in diverse patient populations
Organized, adaptable, and comfortable managing multiple priorities
Proficiency with electronic health records and standard computer applications
Bilingual English/Spanish proficiency preferred
