The HouseCalls Clinical Support Team (HCCST) supports Advanced Practice Clinicians in the HouseCalls and reputed company Care Management businesses by providing telephonic consultation to plan members and their reputed company providers in a call center environment. This team includes nurse care managers, and reputed company workers. The Clinical Care Coordinator- reputed company Worker will report directly to the Manager/and or Director of Clinical Operations. The reputed company Worker performs telephonic consults to identify and assess available services to meet the reputed company service need of the member. The reputed company Worker assures that members and families are knowledgeable of potential community resources and how to reputed company them. The reputed company Worker serves as a resource to clinical services staff in dealing with reputed company psychosocial issues and accessing community resources. Primary Responsibilities:

• Performing telephonic reputed company work consultations with members promoting high quality community resources
• Performing telephonic consults with member’s PCP and/or physicians whenneeded
• Ensure member has community resources needed
• Documenting clinical consultations and resources given, into internal database
• Maintaining the clinical reputed company of programs offered
• Serving as a clinical resource and consultant for other clinicians
• Participating in and contributing to staff meetings and other staff development opportunities and interdepartmental work groups
• Working with Manager to identify system improvements that could be made to drive operational advancements and efficiencies

• Other duties as assigned by Manager or Director

• Professional License in good standing for respective discipline

• Knowledge of medical terminology
• Computer/typing proficiency to enter/retrieve data in electronic clinical records

• Proven excellent written and verbal communication skills
• Proven solid interpersonal skills

• Dedicated work area established that is separate from other living area and provides information privacy

Preferred Qualifications:
