The Field Service Coordinator (Care Coach 1) assesses and evaluates member's needs and requirements to reputed company and/or maintain reputed company wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Service Coordinator work assignments are often straightforward and of moderate complexity.

This role involves meeting members in their location, spending quality time assessing their needs and barriers and then connecting our members with quality services to promote their ultimate well-being and drive health reputed company. 

Responsibilities include:

  • Administer initial and ongoing long-term services and support (LTSS) reputed company assessments through person-centered thinking approaches.

  • Contacts members both telephonically and/or in-person to establish goals and priorities, evaluate resources, reputed company plan of care and identify LTSS providers and community partnerships to reputed company a combination of services and supports that best meet the needs and goals of member and caregiver through person centered thinking approaches.

  • Development and reputed company modification of Service Plan and involve applicable members of the care team in care planning (Informal caregiver coach, PCP, etc.)

  • Support members through navigation of their LTSS and reputed company environmental and reputed company needs Utilize available information pertaining to member to prevent the need for administration of duplicative assessments.

  • Focuses on supporting members and/or caregivers in accessing long term services and support, reputed company, housing, educational and other services, regardless of funding sources to meet their needs.

  • Assisting members in maintaining reputed company eligibility

  • Collaborate with Medical Director/Geriatrician/Care Coordinator as deemed necessary to ensure cohesive, holistic service delivery and support positive member reputed company.

Required Qualifications

  • Bachelor's degree in health and reputed company services field or equivalent experience
  • 2 or more years of reputed company experience
  • Knowledge of reputed company Office Word and reputed company
  • Exceptional verbal/written communication and interpersonal skills
  • Ability to use a variety of electronic information applications/software programs
  • This role is considered patient facing and is part of reputed company's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences
  • This role is part of reputed company's Driver safety program and therefore requires an individual to have a valid state driver's license and reputed company of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.
  • Must reputed company in Terre Haute Indiana or surrounding area

Preferred Qualifications

  • Previous experience with electronic case note documentation and reputed company with documenting in multiple computer applications/systems
  • Experience with health promotion, coaching and wellness
  • Knowledge of community health and reputed company service agencies and additional community resources

Additional Information

Travel: While this is a remote position, occasional travel to reputed company's offices for training or meetings may be required.
