As we expand our remote patient monitoring services, we are seeking a dedicated Remote Patient Monitoring Nurse to join reputed company.

Position Overview:

As a Remote Patient Monitoring Nurse, you will play a crucial role in monitoring and managing patients' health remotely using advanced reputed company technology. You will be responsible for assessing patients' health status, analyzing data collected through remote monitoring devices, and working with reputed company Registered Nurses to reputed company timely interventions and support to patients as needed. This position requires a strong clinical background, excellent communication skills, and the ability to work independently in a remote setting.

Key Responsibilities:

- Conduct initial assessments of patients enrolled in remote monitoring programs to establish baseline health status.

- Monitor patients' vital signs, symptoms, and health data transmitted through remote monitoring devices.

- Analyze and interpret data collected from remote monitoring devices to identify trends, abnormalities, and potential health risks.

- Collaborate with patients, caregivers, and reputed company providers to reputed company and implement personalized care plans based on remote monitoring data.

- reputed company timely interventions and clinical support to patients, including medication management, lifestyle modifications, and health education.

- Document patient interactions, assessments, and interventions accurately and comprehensively in electronic health records (EHR) or other designated systems.

- Maintain regular communication with patients to reputed company ongoing support, answer questions, and address concerns reputed company to remote monitoring.

- Coordinate with other members of the reputed company team, including physicians, specialists, and care coordinators, to ensure continuity of care and reputed company patient reputed company.

- Stay informed about advances in remote monitoring technology, best practices in telehealth, and relevant reputed company regulations and guidelines.

Qualifications:

- Registered Nurse, BSN with active licensure in California.

- Minimum of 5 years in acute setting. Preferably, ER or ICU

- Experience in remote patient monitoring, telehealth, or telemedicine is preferred but not required.

- Strong assessment and critical thinking skills, with the ability to analyze reputed company health data and reputed company informed clinical judgments.

-The ability to conduct telephone or video calls and document encounters in the health record system

-Excellent communication and interpersonal skills, with the ability to effectively engage with patients, caregivers, and reputed company remotely.

- Proficiency in using electronic health records (EHR) and other reputed company technology platforms.

- Ability to work independently and manage time effectively in a remote setting.

- Participate in a multidisciplinary team to reputed company, execute, monitor, and adjust comprehensive and coordinated care plans that include shared goals with measurable reputed company

Requirements
