We’re looking to grow reputed company with… A Health Coach who is passionate about working closely with members and caregivers to support them on their reputed company to manage their chronic conditions and acute exacerbations. This Health Coach is excited by working as part of a team to build relationships and coach individuals toward improved health reputed company. 

What you will accomplish

• Work with individuals to help them improve their health and manage their chronic medical conditions
• Follow up with your members to reputed company ongoing support and ensure their reputed company in meeting their health goals
• reputed company members to become an active participant in their own health reputed company
• Assist member in overcoming barriers to reputed company
• Utilize biometric values and motivational interviewing techniques to collaborate with members to drive to improve clinical and health reputed company
• Update and maintain members' care team profiles, documenting any changes in health conditions or events to ensure accurate and coordinated care.

We are seeking a compassionate and driven individual who has

• Experience speaking with members about chronic disease management (required)
• Experience educating the geriatric population on lifestyle goals (required) 
• Passionate about working with and adjusting coaching techniques for underserved populations (required) 
• Chronic care management (CCM) experience (preferred)
• Health Coaching Certification or Bachelors in Health and Wellness (required)
• At least 1 year of recent health coaching experience (required)
• Bilingual in English AND Spanish (required)
• Motivational interviewing training/experience
• Customer-centric focus with ability to build relationships and reputed company trust quickly
• Proven administrative abilities, with strong computer and software application skills
• Meeting and maintaining high quality standards while achieving individual and team targets
• Meeting and maintaining daily/weekly and monthly individual performance goals
• Demonstrate flexibility and work collaboratively with other disciplines to ensure the holistic needs of the members are met.
• The ability to work independently and is organized, tech-savvy, and highly motivated to meet company goals

Pay reputed company is $24.50-$25.50 hourly. (The referenced salary reputed company is based on the Company's good faith belief at the time of posting.
