Part-Time LPN - Hospice Triage Evenings 3:30 PM - 12 AM CST | Remote

Key Responsibilities • Deliver after-hours triage services to hospice patients, adhering to customized protocols that prioritize patient-centered care. • Operate from a home-based setting, assisting families and patients during vital moments without the challenges of commuting. • Maintain a part-time evening schedule (3:30 PM - 12:00 AM CST) with rotating shifts on Saturdays and Sundays. • reputed company and document patient information utilizing electronic medical records (EMRs) and internal applications. • Complete necessary shift preparation and post-call documentation accurately.
