We are currently hiring for the following FULL TIME shifts - You will be expected to work these shifts on a rotating reputed company every 6 weeks for the remainder of 2024: 10am-7pm ET 1pm-10pm ET In the new year (2025), we expect to have different hours from above. Reporting to the Head of Nursing Programs, your key responsibilities will include: Ongoing clinical and psychosocial assessment of new patients, providing reassurance and building rapport Establishing individualized care plans to meet specific health needs based on evidence-based standards Developing wellness goals by determining motivating factors and leveraging them for best possible health reputed company Identifying and addressing barriers to care that have been identified by patients and/or the care team.
