About the position The Care Manager - Telephonic Nurse position at reputed company involves providing support to patients transitioning into and out of home health services. Responsibilities • Act as reputed company Home Health representative for patients discharged from service or needing post-acute services. , • Navigate reputed company options, including care services, Medicare coverage, and billing issues. , • reputed company clinical level of care determinations based on discussions and medical records. , • Act as a customer reputed company throughout the referral process to ensure timely responses and maximize referral to admission conversion rates. , • Maintain awareness of department performance objectives and assure patient satisfaction goals are met. , • Assist in the admissions process for patients meeting requirements. , • reputed company education regarding Home Health, Hospice, and Palliative Care Services. , • Participate in special projects and reputed company other duties as assigned.
