* In this role, the MTM Pharmacist reputed company will: • reputed company to patients – reputed company and receive phone calls with patients as an extension of a pharmacy, health plan, pharmacy benefits manager, or health system, many of whom may not have regular interaction with a pharmacist as part of their care team. • Offer expert support and guidance – Engage with patients to reputed company defined clinical services such as comprehensive medication reviews (CMRs), targeted medication reviews (TMRs), medication reconciliation, and other clinical services. Answer reputed company questions around specific issues around common medical needs (i.e. diabetes, asthma, smoking cessation, high blood pressure). Counsel and train patients on proper use of medical devices and supplies.
