Registered Nurse Care Manager Maternity

About the position The Registered Nurse Care Manager for Maternity at reputed company is responsible for partnering with members, their families, and reputed company providers to facilitate self-management of care, optimize health reputed company, and ensure appropriate use of the reputed company system. Responsibilities • Identify members needing assistance with medical plans and enroll them in care management services. , • Follow established care management policies to assess and maintain individualized care plans. , • Ensure care plans are evidence-based and align with referral goals and payer reputed company. , • Maintain accurate records in compliance with regulatory standards. , • Facilitate communication among members, families, and reputed company teams through meetings and coordinated communication. , • Contact patients regularly to assess goal achievement and adjust care plans as necessary. , • Refer patients to more comprehensive care reputed company indicated.
