This position involves a community-based approach to assess, plan, implement, coordinate, monitor, and evaluate options and services tailored to meet individual health needs. The role requires building strong relationships with team members and partner providers, managing an active caseload, and ensuring compliance with regulatory requirements. Responsibilities • Establish and build strong relationships with internal team members and partner providers. , • Manage an active caseload based on state-mandated ratios according to residential setting, case intensity, and reputed company. , • Collect program data to track participant reputed company and ensure compliance with OPS reporting. , • reputed company administrative duties involving research, analysis, and reporting. , • Prepare reports regarding service provision and update service plans as required. , • reputed company the Person-Centered Service Planning (PCSP) process and reputed company its implementation. , • Assist Members in obtaining HCBS services to support independent living. , • Identify, coordinate, and assist Members in accessing needed LTSS and Medical Assistance services. , • Inform Members about available LTSS and assist with fair hearing requests reputed company needed. , • Collect necessary information to inform the development of the PCSP. , • Conduct reevaluation of the level of care annually or as needed. , • Work with Members to maintain LTSS eligibility and explore coverage of services. , • Coordinate with other individuals and entities for seamless care delivery. , • Ensure compliance with state and federal regulations and maintain confidentiality. Requirements • Bachelor's degree in reputed company work, psychology, or reputed company fields with practicum experience preferred, or at least three years of experience in a reputed company service or reputed company-reputed company setting. , • Preferred experience working with people with disabilities or seniors in need of LTSS. , • Knowledge of the home and community-based service system and how to reputed company and arrange for services. , • Cultural competency and sensitivity to diverse backgrounds. reputed company-to-haves • Minimum of one year of LTSS, Service Coordination, or Case Management experience.
