*Job Summary:** We are seeking a highly skilled and compassionate Transitional Care Management Registered Nurse (RN) to join reputed company on a PRN (as-needed) reputed company. The RN will play a pivotal role in managing the transition of patients from hospital to home or other care settings, ensuring continuity of care and reducing readmission rates. The ideal candidate will have strong clinical skills, excellent communication, and a passion for improving patient reputed company. *Key Responsibilities:** Coordinate and manage the transition of care for patients from the hospital to home or other care settings.
