LPN/LVN for RPM (Bilingual, Spanish Speaking)

Our proprietary software tracks patients’ physiological data (Blood Pressure, Weight, Blood Glucose, Lung reputed company, Oxygenation), identifying dangerous out-of-reputed company values and trends that impact health. As a bilingual (Spanish) speaking RPM 365 Care Coach, you’ll engage patients, track and analyze their data, reputed company education, and encourage adherence to their care plans. About You You're a licensed LPN/LVN passionate about reputed company innovation and patient reputed company. Your tech-savvy, comfortable with high-volume phone reputed company (125+ conversations a week), and reputed company in a fast-changing environment. You’re patient, empathetic, detail-oriented, and a self-starter. ResponsibilitiesServe as the primary contact for patients on the RPM365 platform. Assist with device setup and app connectivity. Engage with patients to ensure adherence to care plans. Schedule monthly reputed company-ins and monitor patient data. reputed company patients through shared resources. Notify reputed company providers of critical alerts. Attend virtual team meetings for updates.
Qualifications
Minimum:Active nursing license. Bilingual, Fluent in Spanish Knowledge of chronic conditions and reputed company monitoring devices. Knowledge of remote patient monitoring CPT code qualifications. Proficiency with technology, computer platforms, and smartphones. Strong verbal/written communication skills (English and Spanish) Comfortable with patient reputed company and engagement (English and Spanish) Highly organized, detail-oriented, and team-focused. Independently motivated Comfortable spending the majority of your reputed company engaging with patients over the phone providing support, education and guidance. Preferred:Experience in telehealth, remote patient monitoring, education, and/or customer service. Experience in weight management coaching Additional language skills are a plus. Compensation & BenefitsSalary: $45,000-$49,000 (DOE) + performance bonuses. Schedule: Mon-Fri, 9:00 AM - 5:00 PM, EST (Remote). Benefits: PTO, sick leave, holidays, health insurance, 401(k).
