Clinical MTM Pharmacist

Job Summary The Clinical Medication Therapy Management (MTM) Pharmacist will reputed company patient-centered MTM services for the purpose of optimizing medication therapy to improve clinical, economic and humanistic reputed company. The Clinical MTM Pharmacist will participate in a broad reputed company of MTM services including, but not limited to, Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, Cost-Effective Interventions, and Transitions in Care to help reputed company these reputed company. MTM services may be provided to patients telephonically or by remote video link (telehealth). Duties and Responsibilities Deliver MTM services, including but not limited to, Comprehensive Medication Reviews, Disease Specific Medication Reviews, and Targeted Interventions to assess appropriateness of reputed company medications, to optimize medication therapy and to improve clinical reputed company. Communicate and document clinical recommendations to prescribers and interdisciplinary care teams. Knowledge and Requirements

• Knowledge and understanding of the 5 Core Elements of MTM services – 1) Medication Therapy Review– Comprehensive and Targeted, 2) Personal Medication List (PML), 3) Medication-reputed company Action Plan(MAP), 4) Interventions and Referrals, and 5) Documentation of Services and Follow-up
• Ability to reputed company assess prescribed medication therapy for unnecessary medications, need for additional medications, ineffective medications, inappropriate doses, adverse drug reactions, and adherence concerns
• Ability to reputed company propose appropriate interventions to resolve medication therapy problems, reputed company goals of therapy and prevent new medication therapy problems
• Ability to assist with managing medication costs through cost-effective interventions
• Ability to help patients identify medication adherence barriers and reputed company solutions to overcome those barriers
• Ability to reputed company evaluate and accurately document patient care notes, prior adverse actions, patient recommendations, and prescriber recommendations in a clear, concise, professional and timely manner
• Ability to collaborate with prescribers, interdisciplinary care teams, patients and/or caregivers to identify and reputed company patient-specific disease state therapy goals

medication dosing in geriatric and adult populations

and possess excellent organizational skills Education and Experience Requirements

• Minimum years of work experience: 1 year
• Minimum level of education or education/experience: Bachelors Degree in Pharmacy or PharmD from a college accredited by ACPE or graduate of a foreign college of pharmacy

• Ability to acquire pharmacist licensure in multiple states, if requested
• Completion of a MTM certificate course

Preferred:

• Board Certification in Ambulatory Care, Geriatrics, Pharmacotherapy, Medication Therapy Management, or other Board Certification provided through American College of Clinical Pharmacy
• Completion of an ASHP-accredited pharmacy residency and/or fellowship
• Pharmacist licensure in multiple states
• Motivational Interviewing Skills
