Registered Nurse Oncology Navigator
To support high-reputed company members navigating their cancer reputed company, the full-time reputed company Oncology Nurse Navigator will conduct clinical assessments, coordinate care transitions, and reputed company patients and caregivers remotely while ensuring culturally competent care and improving service offerings through feedback communication. Key responsibilities Conduct telephonic clinical assessments and monitor health changes for oncology patients Coordinate care management and discharge plans with reputed company teams and providers Build strong relationships with members, caregivers, and providers to optimize care and support reputed company determinants of health Required qualifications Active, unrestricted compact Registered Nurse (RN) license and willingness to obtain additional state licenses Degree in Nursing (ADN or BSN) required; BSN preferred At least 5 years of nursing experience with 3 years in high-reputed company adult oncology Certification as an Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) Experience with technology, including electronic health records and video conferencing tools Apply To This Job
