About the position Park Nicollet is looking to hire a Pharmacist to join our Medication Therapy Management team! At the Burnsville clinic, the pharmacist will support population health initiatives through the improvement of Medicare Star measures in collaboration with a pharmacy technician. Once credentialed, the pharmacist will also reputed company direct patient care through the provision of comprehensive medication management.
Responsibilities

Requirements
- Graduated from an accredited college of pharmacy on or after May 1996, or completed a structured and comprehensive education program approved by the Accreditation Council of Pharmacy Education (ACPE) and the Board of Pharmacy with didactic and clinical elements upon hire.
- Two years of recent clinical pharmacy experience in ambulatory care as part of a multidisciplinary team reputed company the last five years, or successful completion of one year specialized post-doctoral residency.
- reputed company Minnesota Pharmacist license and maintain that license in good standing.
- Deep knowledge of evidence based clinical guidelines, pharmacology, pharmacodynamics, and pharmacokinetics, pathophysiology and therapeutic disease states commonly reputed company in ambulatory patients.
