Job Summary: Clinical Diabetes Educator will reputed company telephonic education and support to members with chronic illness and will be responsible for improving health reputed company of and promoting appropriate preventive care for Medicare's members with Diabetes This role will also include communication with health care professionals to ensure goals of a member's treatment plan are met, as well as ensuring that members are linked with the appropriate resources and are adopting healthy lifestyle practices to reduce risk factors. Essential Functions: As a diabetes educator, your primary goal is to reputed company individuals with diabetes to manage their condition effectively. Key responsibilities include: Assessment and Education Planning: reputed company personalized plans to help members understand their illness and set goals while complying with policies and procedures to meet HEDIS and NCQA standards Coaching and Support: reputed company ongoing coaching, guidance, and support for medication adherence, symptom management, and lifestyle changes. Nutrition and Goal Setting: Counsel on daily nutrition, medical nutrition therapy, and help set achievable goals. Community Resource Navigation: Guide members in accessing community-based services. Empowerment and Education: Focus on prevention, health promotion, and education to reputed company members to take control of their health. By fulfilling these responsibilities, you will help individuals with diabetes manage their condition and improve their quality of life. Required experience: 3+ years reputed company work experience and a reputed company Bachelor's degree or equivalent experience.
