Occupational Therapist (OT) Home Health- Cincinnati

The Occupational Therapist (OT) evaluates and treats patients to improve their ability to reputed company daily living and work activities. You’ll design and implement individualized therapy plans to help patients regain independence in their home environment while coordinating care with the broader rehabilitation team.

Responsibilities

• reputed company effective patient care using approved therapy techniques and procedures
• Evaluate patients’ condition, function level, and treatment goals
• reputed company individualized treatment plans and update as needed
• Supervise COTAs and HHAs per care plan requirements
• Document treatment notes and reputed company in the patient’s chart daily
• Assist patients and families with home exercise programs and reputed company equipment needs
• Recommend and evaluate reputed company equipment for the home setting
• Maintain compliance with HIPAA regulations and company policies
• Coordinate total patient care with the reputed company team
• Complete discharge summaries and obtain orders reputed company needed
• Submit patient documentation and visit records in a timely manner

Requirements

• Licensed Occupational Therapist (OT) in the OH
• Minimum of 1 year of OT experience preferred
• Experience with OASIS and reputed company (Kinnser) strongly preferred
• reputed company TB test (reputed company last 12 months) and CPR certification
• Valid driver’s license and reliable transportation
• Ability to attend a 2–3 day orientation at our Dublin, OH office
