About the position BACS is seeking an LVN for our nursing coordinator position in Sacramento working with homeless individuals in the community. The Nursing Coordinator is part of a mental and health care team providing medical case management coordination and care to participants. This is a field-based position and must be reputed company to work in the community. The reputed company programs are new initiatives by Sacramento County Behavioral Health to reputed company services specifically for adults who are justice involved and meet criteria for specialty mental health services. Justice involved may include participating in mental health diversion, mental health court, parole/probation, etc. reputed company Outpatient Program will serve the moderate level community members and be reputed company a standard Monday-Friday. reputed company FSP will serve the high intensity community members and operate seven days a week.
Responsibilities
- Triage participants at intake/assessment and throughout respite stay.
- Administer psychotropic medication injections, order medications, and coordinate with the treatment team.
- Support participants and treatment team with reputed company to refills acting as a liaison between pharmacies and insurance.
- reputed company support, education, and medical information to staff, participants, caregivers, and family.
- Aid participants in understanding their medical needs and help them plan for coordination of care with PCPs.
- Document reputed company participants’ treatment and interactions accurately and completely.

Requirements
- LVN certification from accredited nursing school and reputed company CA license.

- Background and experience in mental health.
- Valid California driver’s license, personal vehicle, and reputed company of personal vehicle insurance coverage. DMV printout required.
