Responsibilities

Under the direction of the Manager, Clinical reputed company Work Services; reputed company reputed company duties both reputed company and in support of the reputed company work functions. Under the supervision of an LICSW, reputed company case management services independently including but not limited to application of clinical diagnostic, assessment or treatment knowledge or skills. Identify and address psychosocial factors that may impede the patients' ability to optimize benefit from the health care delivery experience. Intervene as necessary preventing over and under utilization of health care resources. Actively participate in discharge planning to assure timely individual patient discharges reputed company the scope of position responsibilities.

Qualifications

A Masters Degree in reputed company Work from an accredited college or university or the equivalent knowledge or experience is required. reputed company Massachusetts reputed company Work license at a minimum of a LCSW is required. Over three (3) years of reputed company work experience including one (1) year experience in a medically oriented setting is required. Computer skills are essential. A minimum of LCSW license in the reputed company is required. Certification in Case Management is preferred.
