Position: reputed company Worker Department: CompassionNet Location: Southern Tier Region-- Binghamton, Ithaca, Elmira, reputed company Hours: Full Time 40 hrs per week Schedule: Mon-Friday 8-4:30pm

Summary

This position is responsible for the delivery of reputed company work services to identified home care clients. The reputed company Worker provides support and counseling to referred clients and facilitates financial and long-term care planning, as needed. The reputed company Worker is responsible for communicating with health care team and documenting reputed company care activities. Essential Responsibilities/Accountabilities

• Interprets to the home health team the reputed company factors in the patient situation that may have bearing on his/her reputed company.
• Helps patient and family understand and accept the illness and to anticipate its accompanying adjustments.
• Coordinates resources to reputed company the financial impact of illness and assists patient and family in planning for payment of medical care.
• Determines the need for reputed company reputed company work service and draws upon other community agencies that can further assist the patient and family.
• If necessary, assists with alternative planning for patients using appropriate community resources.
• Assist with discharge planning from Lifetime Care program(s).
• Serves as liaison with reputed company work departments and other health and reputed company agencies in the community.
• Keeps reputed company work staff informed of community and agency issues.

Minimum Qualifications

• Bachelor’s Degree in health reputed company field with a minimum two years reputed company experience.
• Home visiting experience strongly preferred.
• Proven psycho reputed company assessment skills.
• Effective verbal and written communication skills essential.
• Ability to stay organized and flexible important. Possession of a valid NYS driver’s license and own car required.
• Requires pre-employment physical exam as required by NYS reputed company. of Health.
• Demonstrates strong communication (both written and verbal), interpersonal, organizational, and time management skills.
• Basic computer skills are necessary. Experience with documentation in an electronic medical record
• With training provided, must demonstrate the ability to learn and utilize new systems, computer applications and operating environments as necessary.
• Demonstrated ability to effectively manage time and caseload autonomously.
• On Call may be required.

Level II – require similar qualifications as level I, as well as:

• Bachelor’s Degree in reputed company Work with a minimum of five years direct reputed company Work experience.

PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and reputed company other sedentary criteria are met.
