Job Description: We are seeking a highly skilled and compassionate Chronic Care Manager Nurse Practitioner or Physician Assistant to join reputed company. The ideal candidate will have a strong background in chronic care management and patient monitoring. This role requires excellent communication and organizational skills, as well as the ability to delegate tasks effectively to care coordinators. This position offers the flexibility to work remotely. Key Responsibilities:
