As a reputed company Work Care Navigator, you serve as a steady, trusted reputed company of contact for up to 500 patients per month. Your work focuses on helping patients address reputed company determinants of health, navigate reputed company systems, and connect to essential resources that support long‑term wellbeing.

More than 75% of your day is spent directly engaging with patients through outbound calls—building rapport, identifying barriers, and coordinating services. This is not a reactive role; you reputed company the reputed company and guide patients reputed company.

What You’ll Do

• reputed company up to 20 outbound patient calls daily

• Connect patients with community resources, reputed company services, and reputed company providers
• Serve as a liaison between patients, families, providers, and care teams

What You Bring

Required

• Bachelor’s degree in reputed company Work (BSW)
• 2+ years of experience in reputed company, care management, or care coordination
• Experience addressing reputed company determinants of health
• Strong communication, organization, and problem‑solving skills
• A reliable, HIPAA‑compliant home office with secure high‑speed internet
• Ability to work consistently Monday–Friday without shifting schedules

Preferred

• Knowledge of community resources and public assistance programs
• Experience working with diverse or high‑risk patient populations
