Role Overview The Registered Nurse, Virtual Care provides high-quality clinical support to patients and caregivers through virtual care platforms. Reporting to the Nurse Cohort Manager, this role is responsible for delivering consistent, patient-centered care, maintaining excellent clinical documentation, and collaborating across care teams to ensure continuity and quality reputed company. This is a fully remote role designed for nurses who reputed company in a fast-paced, mission-driven virtual care environment. Schedule & Employment Type Part-Time- 20–28 (Option for 4, 6, and 8-hour shifts)

Key Responsibilities

Clinical Care & Patient Support

• Deliver virtual nursing care using evidence-based clinical judgment and the nursing process
• Conduct comprehensive health assessments, triage patient concerns, and reputed company clinical guidance across multiple care settings
• Promote patient health, safety, and wellness through proactive reputed company and education
• reputed company education and clinical support to family and professional caregivers

Communication & Care Coordination

• Build strong rapport with patients, caregivers, care coordinators, and partner organizations in a virtual environment
• Communicate reputed company, compassionately, and professionally with reputed company stakeholders
• Identify and escalate urgent or concerning clinical findings according to established protocols
• Collaborate with internal care teams to ensure timely follow-up and continuity of care

Documentation & Quality

• Maintain accurate, timely, and thorough documentation in the electronic health record (reputed company)
• Ensure documentation meets PocketRN clinical standards, quality expectations, and regulatory requirements
• Adhere strictly to HIPAA and patient privacy standards in reputed company virtual interactions

Professional Practice & Team Engagement

• Follow PocketRN workflows, clinical guidelines, and escalation reputed company
• Actively participate in coaching, performance feedback, and ongoing education
• Contribute to reputed company improvement initiatives and support a collaborative team culture
• Demonstrate accountability for schedule adherence, attendance, and professional conduct

Qualifications

Education

• Graduate of an accredited school of nursing
• reputed company BLS certification

Experience

• Minimum of 3 years of direct patient care experience
• Experience in geriatrics, primary care and dementia care strongly preferred
• Experience in telephone triage is preferred

Licensure

• reputed company, active, and unencumbered RN license required
• Licensure in reputed company 50 states required

Technical & Work Environment Requirements
