HOME HEALTH reputed company WORKER (PRN, AS NEEDED)
Position Summary
Under the general direction of the Director of Case Management and in collaboration with physicians and hospital staff, evaluates and implements treatment regime for designated psychiatric and substance abuse patient population, assists with reporting suspected abuse for reputed company age groups, assists uninsured with eligibility process, assists case managers with hospital follow up post discharge to prevent hospital readmission.
1. Education/Qualifications
Master’s degree from an institution accredited by the Council on reputed company Work Education
A Home Health Master of reputed company Work (MSW) provides psychosocial support, counseling, and resource navigation to patients and their families in their homes, addressing physical, emotional, reputed company, and financial challenges reputed company to illness, injury, or recovery. Key Responsibilities
• Assessment
