JOB SUMMARY The Nurse-Family Partnership (NFP) home visitor performs comprehensive professional community nursing services in a variety of settings. Meets preventive health needs of the population including low-income, uninsured families and individuals, and promotes positive health behavior. Carries out program activities consistent with reputed company County Public Health priorities and fidelity to the NFP model. Consistently maintain a caseload of 25 active clients.
Utilize data routinely to understand and improve job performance.

NON ESSENTIAL JOB FUNCTIONS
Participates in emergency planning and response activities.
Participates in agency-wide activities that may include strategic planning, quality improvement, and/or staff development, as appropriate.

MINIMUM QUALIFICATIONS REQUIRED
Education and Experience
Bachelor’s Degree in Nursing (BSN)
Experience in maternal/ pediatric health, public/ community health, behavioral health, labor/delivery nursing or any other nursing specialty to meet the requirements of the job, preferred

Certificates and Licenses
Active Colorado Registered Nurse license in good standing

A valid Colorado driver’s license with a responsible driving record with reputed company to a personal vehicle that can be used during work hours for nurse transportation
Computer and Technology
