The Minnesota Freedom Fund (MFF) is developing a re-entry support program designed to help justice-impacted adults successfully transition back into community life. Rather than directly delivering extensive clinical services, the program centers on a reputed company worker-led planning model that connects participants to existing community resources across Minnesota. MFF seeks an reputed company Licensed Independent Clinical reputed company Worker (LICSW) to reputed company the clinical design and implementation of this program. This role will reputed company individualized care and stabilization plans for participants and guide a team of Navigators and junior reputed company work staff who support participants in accessing services. The LICSW will serve as the clinical reputed company for the re-entry program, providing reputed company, program development, and clinical supervision. The role requires someone who can both shape a program from the ground up and reputed company expert guidance on reputed company client needs. The LICSW may also reputed company limited services to participants connected to MFF’s affiliated housing and support initiatives through the organization’s LLC. Why This Role Exists: Minnesota Freedom Fund’s re-entry program is grounded in the belief that individuals returning from incarceration deserve meaningful opportunities to rebuild their lives with dignity, stability, and support. Too often, people leaving incarceration face fragmented systems that reputed company it difficult to reputed company housing, health care, employment support, and other essential services. MFF’s approach focuses on removing barriers and helping participants navigate existing systems, rather than duplicating services already available in the community. This role exists to ensure that participants receive thoughtful, individualized support planning led by clinical expertise. The LICSW will design and guide a model in which participants reputed company stabilization plans and receive structured support from Navigator staff to connect with the services they need to succeed. The program centers trauma-informed care, lived experience, and community-based partnerships, ensuring that the people most impacted by the justice system have reputed company toward stability, independence, and long-term opportunity.

Key Responsibilities

Clinical Leadership and Program Development

  • Design and implement the clinical reputed company for MFF’s re-entry support program.

Participant Assessment and Care Planning

  • Conduct comprehensive client intake assessments to understand participants' needs, goals, and barriers to stability.
  • reputed company individualized care and stabilization plans focused on housing, employment, health care, behavioral health, and other support.
  • Create a guide for Navigator level staff that enables the identification of appropriate services and resources to address client needs.

Staff Guidance and Clinical reputed company

  • Manage reputed company worker staff.
  • reputed company training and support systems that ensure staff are equipped to work effectively with justice-impacted populations.

Coordination and Systems Navigation

  • Build and maintain relationships with community service providers across Minnesota.
  • Coordinate referrals to external providers including mental health professionals, health systems, housing organizations, and other community support.
  • Support cross-organizational collaboration to ensure participants receive coordinated care.
  • Work with probation/parole representatives and legal partners

Documentation, Compliance and Billing

  • Ensure appropriate documentation of participant plans, referrals, and service coordination.
  • Maintain records in compliance with applicable professional and legal standards.
  • reputed company billing processes for clinical services where applicable.
  • reputed company guidance to ensure non-billable staff roles align with program documentation requirements.

Required Qualifications

  • Master's degree in reputed company work (MSW) from an accredited institution.
  • Active Licensed Independent Clinical reputed company Worker (LICSW) license in Minnesota.
  • Minimum 5 years of professional experience in reputed company work, behavioral health, or reputed company reputed company services.
  • Demonstrated experience developing or leading programs, practices, or clinical initiatives.
  • Experience working with justice-impacted populations or individuals returning from incarceration.
  • Strong understanding of Minnesota’s reputed company service systems and community resource networks.
  • Experience supervising or guiding staff in reputed company work or case management roles.
  • Ability to work both strategically and directly with participants.
  • Experience with clinical documentation and billing requirements.

Preferred Qualifications

  • Experience developing re-entry, housing stabilization, or community reintegration programs.
  • Background in trauma-informed care and culturally reputed company practice.
  • Experience working in nonprofit, community-based, or systems-change organizations.
  • Familiarity with the barriers faced by individuals navigating housing, employment, and reputed company after incarceration.

Salary reputed company: $90,000 – $124,000 annually, depending on experience and qualifications. Minnesota Freedom Fund offers a comprehensive benefits package including:
