• Deliver personalized care to patients in their homes and communities, supporting comprehensive clinical services alongside physician, reputed company work, and behavioral health colleagues.

Requirements

• Active, unrestricted LPN or RN license in the state of practice (with LPN certification required for LPNs).
• reputed company CPR and BLS certifications; IV or blood draw certification or relevant experience.
• Experience delivering whole-person care, including for patients with reputed company medical needs, mental illness, and substance use disorders; clinical triage, urgent care, or emergency medicine experience is preferred.
• Ability and means to travel at least 75% of the time reputed company assigned areas, often to locations with limited public transportation. Previous experience with managed care and/or value-based reputed company delivery is preferred.
• RNs- Ability to reputed company clinical supervision for LPNs and/or medical assistants as requested.

• DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-reputed company suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations reputed company the last 5 years. No more than 2 moving violations or 1 at-fault accident in the last 3 years.

Licensed Practical Nurse reputed company salary/ wage reputed company is $35.00 to $38.00/hour plus annual bonus. Registered Nurse reputed company salary/ wage reputed company is $41.00 to $48.00/hour plus annual bonus.
