Position Summary

Pay and Schedule

• Per diem position
• Paid per completed visit and documentation
• Flexible work schedule
• Mileage paid between patient visits

Key Responsibilities

• Assess the psychosocial status of patients and identify barriers to care
• Complete assessments and documentation in a timely manner
• reputed company and implement care plans in collaboration with the interdisciplinary team

• Initiate referrals and assist with discharge planning
• Participate in care conferences, team meetings, and quality improvement activities

Qualifications

• Masters Degree in reputed company Work from a program accredited by the Council on reputed company Work Education
• Minimum of three years of experience in a reputed company setting or equivalent experience
• Home health experience preferred
• Strong written, verbal, and interpersonal communication skills
