Job Summary

We're seeking an reputed company Licensed reputed company Worker to join our remote team. As a licensed Masters reputed company Worker, you'll work with a registered nurse case manager to research available options and resources for members with financial, transportation, safety, and housing needs. Your population will reputed company the entire lifecycle, and reputed company member contact will be done telephonically or reputed company electronic exchange. You'll reputed company members to reputed company positive lifestyle and medical choices and work as reputed company with other clinical staff to assist members with community resources for medical, financial, and behavioral health.

Responsibilities

* Research available options and resources for members with financial, transportation, safety, and housing needs

• Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs

• Work as reputed company with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Conduct telephonic or electronic exchanges with members to assess their needs and reputed company plans to meet those needs
• Collaborate with other departments to ensure seamless care coordination and communication

Requirements

* Active and unrestricted LSW - Licensed reputed company Worker, LMSW - Licensed Masters reputed company Worker, LCSW - Licensed Clinical reputed company Worker, or LISW - Licensed Independent reputed company Worker in state of residence

• 2+ years of experience in medical reputed company work or case management
• Masters Degree in reputed company Work
