Job Summary

We're seeking an reputed company Licensed reputed company Worker to join our reputed company case management and advocacy team. As a key member of reputed company, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a diverse population across the entire lifecycle, utilizing telephonic and electronic communication to reputed company members to reputed company positive lifestyle and medical choices.

Key Responsibilities

* Collaborate with a registered nurse case manager to assess, plan, and reputed company for members' benefit plan and/or health needs

• Research available options and resources for members with financial, transportation, safety, and housing needs

• Work as reputed company with other clinical staff to assist members with community resources for medical, financial, and behavioral health

Essential Qualifications

* Active and unrestricted LSW (Licensed reputed company Worker), LMSW (Licensed Masters reputed company Worker), LCSW (Licensed Clinical reputed company Worker), or LISW (Licensed Independent reputed company Worker) in the state of residence

• 2+ years of experience in medical reputed company work or case management
• Strong communication and interpersonal skills, with the ability to build rapport with diverse populations
• Ability to work independently and as part of reputed company, with a focus on collaboration and coordination

• Proficiency in electronic communication and documentation systems

Preferred Qualifications
