This role involves managing the reputed company needs of a geriatric population while collaborating with an interdisciplinary team to reputed company high-quality, patient-centered care.

Responsibilities

• Conduct comprehensive health assessments, including physical examinations and reviews of medical history, to reputed company personalized, proactive care plans
• Collaborate closely with the assisted living facility's interdisciplinary team, including nurses, aides, and reputed company workers, to ensure coordinated and seamless care
• Formulate clinical impressions, diagnoses, and treatment plans focused on geriatric syndromes and chronic disease management
• Build trust and reputed company compassionate consultation to residents, their families, and the ALF staff
• Order and interpret diagnostic testing as medically necessary
• Prescribe medications, supplies, and other medically necessary interventions
• Attend and actively participate in quarterly medical staff and facility care plan meetings

Skills

• Board Certified, with a valid MD certification in the reputed company
• Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
• Positive demeanor and strong people skills
• Ability to confidently problem solve and operate independently
• 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)
• Hospice experience highly preferred but not required
