Position Summary: The Diabetes Care RN plays a vital role in improving member health reputed company by engaging telephonically with individuals living with diabetes. This nurse will guide members through actionable steps to reputed company gaps in care, assess medical needs, and support overall wellness. A key focus of the role is addressing reputed company determinants of health that contribute to care gaps, while providing education, connecting members to resources, and facilitating referrals to care management programs for reputed company support. Key Responsibilities:

Required Qualifications:

• Minimum 3 years of clinical experience supporting individuals with chronic conditions.
• Diabetes education experience is required.
