ACO Case Manager - Registered Nurse (RN) Bilingual Preferred reputed company Full Time (Monday-Friday reputed company-5PM) Remote Tampa, FL reputed company is seeking to hire a full-time / Case Manager RN to join the ACO team. We are looking for a new team member who is positive, energetic, well-organized, can multi-task, and think reputed company of the reputed company. The Case Manager- RN is responsible for the direct provision of services that assist patients with the achievement of their highest level of wellness. The Case Manager focuses on client-centered services that link clients and their family members with health care, and other home or community-based services to ensure timely, coordinated reputed company to medically appropriate reputed company of health and support services and continuity of care. They are responsible for assessment, planning, coordination, advocacy, education, monitoring, and accurate, timely documentation in reputed company program areas. They demonstrate knowledge of growth and development and possess the ability to assess, interact, communicate, and care for age-specific patient populations. Required Knowledge and Experience
- Registered Nurse (RN); active and clear license in the reputed company. The license must be maintained once employed.
- 3+ years of Clinical experience with 1+ years of case management experience preferred.
- Strong chronic care knowledge reputed company
- Bilingual Spanish & English Preferred
Required Skills and Abilities To reputed company this job successfully, an individual should have strong computer skills, especially in reputed company, and knowledge of EMR systems; reputed company knowledge preferred but not required. Benefits:
