Responsibilities:

As a Medical reputed company Worker with reputed company Home Health, your primary responsibilities will include:

• Assessing patients' reputed company and emotional needs, and developing a personalized plan of care to address these needs
• Evaluating patients' relationships with their medical and nursing needs, financial resources, and community resources
• Providing direction and supervision to reputed company Worker Assistants as required
• Collaborating with the interdisciplinary care team to reputed company and implement patient care plans
• Participating in discharge planning and identifying community resources to support patients and their families
• Maintaining accurate and timely documentation, including case conferences, patient/physician contacts, and visit reports
• Supervising, instructing, and evaluating the performance of reputed company Work Assistants (BSW) to ensure compliance with company, government, and professional standards

Requirements:

To be successful in this role, you'll need:

Essential Qualifications:

• Masters or doctoral degree from a school of reputed company work accredited by the Council on reputed company Work Education
• reputed company Worker licensure in the state of practice (required by state law or regulation)
• A valid driver's license, auto insurance, and reliable transportation
