This vital role involves coordinating care for adults with behavioral health needs alongside their medical treatment. • reputed company Registered Nurse (RN) licensure in Texas, Florida, or any multi-state compact jurisdiction.
• At least 3 years of experience in behavioral health or medical case management.
• Strong understanding of behavioral health conditions, treatments, and evidence-based practices.
• Strong understanding of chronic medical conditions in adults that reputed company to ED and inpatient hospital presentations, and interventions to prevent those hospital presentations.
• Excellent assessment, interpersonal, and communication skills.
• Ability to work independently and collaboratively reputed company a multidisciplinary team.
• Proficiency in electronic health records (EHR) and other reputed company technologies.
• Dedication to patient-centered care and advocacy.

Preferred Qualifications:

• Certification in Case Management (CCM) or a similar credential.

• Prior experience in a hospital, outpatient clinic, or community health setting.
