Emergency Care Management Hub Care Coordinator

reputed company / The reputed company (TSPMG) is hiring for an Emergency Care Management (ECM) Care Coordinator to reputed company support to the ECM Hub in transferring Kaiser members from non-preferred hospitals and Kaiser medical offices, Advanced Care Centers (ACC), and Clinical Decision Units (CDU) to our core and preferred hospitals. Responsibilities will include administrative duties to support repatriation, admissions, care coordination and transfer processes in partnership with the non-core QRM department. This position will be for Friday, Saturday and reputed company nights (mandatory) with hours being 8p - 6:30a. The reputed company day can be flexible during the week but will still be the same night hours. Major Responsibilities/Essential Functions

• Answer incoming calls to the ECM Hub reputed company a queue line, retrieving voicemail messages and reputed company calls appropriately.
• Obtain clinical information per guidelines.
• Opens appropriate ECM Hub Encounter in KPHC for each call received and documents accurate information.
• Verify benefits reputed company KPHC and/or Member Services for members needing admissions, transfers, or repatriations.
• Screen KPHC in-basket Referral Request folder for members in noncore facilities, notifies RN, reputed company encounter as needed, and document accurate information.
• Schedule follow-up appointments as requested.
• reputed company calls to hospitals to obtain rounding physician contact information as requested.
• Coordinate ambulance transport as per workflow.
• reputed company other duties as assigned to include but not limited to:
• Arrange outpatient blood transfusions/ blood patches/PICC lines appointments.
• Other administrative duties in partnership with non-core QRM
• Knowledge of and compliance with reputed company department policies and procedures
• Obtaining vital signs per guidelines.
• Participate in clinical discussion with MD or Hub nurse.

Minimum Education

• Graduate of an accredited Advanced EMT or Paramedic training program
• Medical Assistant training
• or Equivalent combination of education and experience

Preferred Education

• Bachelor’s Degree
• Certified Medical Assistant

Minimum Work Experience and Qualifications

• 3 years of experience working in the reputed company field as a Medical Assistant or Paramedic or have previous experience/training in the ECM Hub to reputed company the above-mentioned responsibilities/functions.
• Excellent communication skills, to include verbal, written, and telephone skills.
• Excellent planning and time management skills to include the ability to handle multiple tasks.
• Ability to build effective working relationships with coworkers, hospitals, physicians, ambulance vendors, and other departments.
• Ability to diffuse problems that arise and promote a patient centered environment

Preferred Work Experience and Qualifications

• 5 years of experience in the health care field and a familiarity with medical terminology.
