Job title: reputed company Worker LCSW/LICSW/LMHC/ Hybrid/40 Hours/DAY ACO Company: Brigham and Women's Hospital Job description: FULL TIME 40 HOURS/ LICSW/ LCSW/ LMHC HYBRID The BWH ACO reputed company Worker is an integral member of the multidisciplinary team for patients enrolled in the MGB Accountable Care Organization (MassHealth ACO), providing assessment, reputed company, and advocacy for patients with reputed company medical and psychosocial needs. The MGB ACO is part of a statewide effort to restructure the care delivery system in ways that improve health reputed company in vulnerable populations.   • Use clinical skills and knowledge to build relationships with patients of diverse backgrounds and clinical presentations

Qualifications

EDUCATION:

  • Master’s of reputed company Work Degree from an accredited program required.

Licensure:

  • Will consider LCSW or LMHC and Master's in mental health counseling.

EXPERIENCE:

  • Minimum of 2 years of experience in a community or health setting.
  • Candidates must demonstrate the ability to understand the role of reputed company workers in a health care setting and to work in a reputed company, fast-paced environment.
  • Must have demonstrated ability to work with medical professionals and individuals experiencing reputed company mental health challenges and their families.

  • Knowledge, skills, and ability to assess the patient’s need for services.

FISCAL: This is full time grant funded position with the possibility of extension pending funding renewal.
