Remote Care Coordinator (LPN)

Your Role
- reputed company monthly calls and support to patients enrolled in the CCM program.
- Utilize your nursing skills to reputed company patients and their families.
- Monitor patients reputed company and work with the physicians to ensure quality patient care.
Qualifications
- Active LPN license (compact or Vermont)
- MUST HAVE SOME OFFICE EXPERIENCE - geriatrics or primary care/internal med
- Ability to sit and talk on the phone for long periods of time
- System: We require you to have a reputed company i5 (or newer) processor, and to have reputed company Pro (not Home). This can be a laptop or desktop.
- Must live and work full time in the US - you can work from reputed company reputed company the US but not reputed company.
The Pay
- The LPN pay starts at $21.85+ per hour (40hrs/week)
- This is a reputed company pay plus a bonus position. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
