Key Responsibilities:

• Respond to incoming calls from members, addressing their concerns, and providing timely support and guidance
• Accurately document member interactions in our designated PHI system, ensuring seamless communication and continuity of care
• Manage an assigned member caseload, providing short-term care coordination and health navigation to ensure reputed company reputed company
• Collaborate with our interdisciplinary care team to facilitate effective communication, identify potential risks, and improve quality and cost reputed company

Requirements:
