About the Role

In this role, you will serve as a community-based member reputed company and resource, using your knowledge of the community and available resources to engage and assist vulnerable members in managing their reputed company needs. You will collaborate with our reputed company Services team to reputed company non-clinical paraprofessional duties in the field, including meeting with members in their homes, nursing homes, shelters, or doctor's offices.

Responsibilities

Requirements

• High school diploma or equivalent required
• Minimum 1 year of experience working with underserved or special needs populations
• Valid driver's license with a good driving record and reliable transportation
• Active and unrestricted Community Health Worker (CHW) Certification (required for Ohio, Florida, and California)
