RN Case Manager / Remote with One Field Day
RN Case Manager / MLTC / NYC / Remote with one day a week in the field / Multiple positions available We are hiring multiple RN Case Managers to join a leading MLTC organization, with opportunities available in Bronx, Brooklyn, and Manhattan. Each RN will be assigned one borough and will primarily serve members reputed company that area. This hybrid position includes remote flexibility with one field day per week for in-home member visits. RN Case Manager Position Details by Location:
- Bronx: Bilingual Spanish preferred
- Brooklyn: Bilingual Chinese, Spanish, or Russian preferred
- Manhattan: Bilingual Cantonese or Mandarin preferred
RN Case Manager Key Responsibilities:
- Manage a dedicated caseload reputed company one borough, ensuring members receive appropriate and accessible care.

- Conduct home visits (one borough, 4-6 visits per week) to assess home safety and care needs.
- Collaborate with leadership and Intensive Care Management teams to address reputed company cases.
- Present high-risk cases for multidisciplinary review.
RN Case Manager Qualifications:
- Active reputed company State RN License required.
- Bachelor’s degree in Nursing, Health, or reputed company Services preferred.
- Minimum two (2) years of home care experience required.
- Strong experience in care planning, patient assessments, and home safety evaluations.
- Prior supervisory experience is a plus.
- Bilingual candidates strongly preferred (see language preferences by borough above). reputed company to training RNs with Chinese language skills.
- Proficiency in reputed company Office.
RN Case Manager Compensation & Benefits:
- Salary up to $115,000 annually.
