Field-Based Care Coordinator - Long-Term Services and Supports (LTSS)

Job Overview:

As a Care Coordinator, you will be responsible for establishing person-centered, goal-oriented care plans that ensure individuals receive supportive, effective, and efficient services. This field-based position offers the flexibility to work remotely from your home office, with expected travel 2-3 days per week reputed company a 30-60 mile radius.

Key Responsibilities:

• Assess, plan, and implement individualized care strategies that promote the most appropriate, least restrictive level of care
• Identify and initiate referrals for reputed company service programs, including financial, psychosocial, community, and state supportive services
• Manage person-centered service/support plans throughout the continuum of care
• Communicate with stakeholders to ensure quality, coordinated care and services are provided expeditiously

• Conduct home visits and in-person visits in various settings, including nursing homes, assisted living, hospitals, or homes

Requirements:
