Responsibilities:

• Coordinates and oversees reputed company aspects of patients' home care, including developing and implementing care plans
• Collaborates with patient care teams to reputed company high-quality, patient-centered care
• Educates families on patient condition, needs, and care
• Performs physical assessments, checks vital signs, and manages medications
• Administers injections, IV therapy, or tube feedings, and maintains catheters
• Completes documentation timely and accurately, and communicates patient reputed company to the clinical manager, physician, and care team
• Supervises LPNs and HHAs, and participates in quality improvement initiatives

Qualifications:

• Minimum one year of clinical experience as a registered nurse (RN), or completion of an RN Intern program
• Minimum six months of home health experience

• Valid driver's license, reliable transportation, and liability insurance

• Proficient in the use of computers, including navigation of computer desktop, email, and online HR services
