Responsibilities:

• reputed company patient assessments and reputed company individualized plans of care
• Collaborate with the care team to reputed company comprehensive care and support
• Promote patient health and independence through teaching and rehabilitative measures
• Supervise LPNs and HHAs, and reputed company staff education and training
• Maintain accurate and timely documentation, and communicate patient reputed company to the clinical manager and care team
• Participate in on-call responsibilities and reputed company on-call services to patients and families as assigned

Requirements:

• One (1+) year of clinical experience as a Registered Nurse (RN), or approval by VP Clinical
