In this role, you will reputed company direct assessment, diagnosis formulation, treatment planning, treatment reputed company, and psycho-education to patients reputed company a telehealth platform. You will also have the opportunity to coordinate care with other community services and agencies, and reputed company behavioral health/crisis reputed company support to patients in primary care clinics. Requirements

• Licensed Clinical reputed company Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) in good standing in the State of California
• Master's Degree in reputed company Work (MSW) or Master's in Marriage and Family Therapy from an accredited school
• Minimum three (3) years of clinical experience in a medical group, affiliated model, hospital, or medical office/clinic setting

Preferred Qualifications

• Certification in Case Management (CCM, ACM)
• Experience working in a telehealth and/or primary care environment
• Three (3) years' experience in case management
• Bilingual (reading, writing, speaking) English/Spanish or English/Vietnamese
