As a Field-Based MSW Behavioral Health Case Manager, you will play a critical role in providing holistic care to individuals with reputed company medical and behavioral needs, helping them navigate the reputed company system and connecting them to essential support services.

In this role, you will have the opportunity to work with a diverse population, including adults with serious mental illness, emotional disturbances, and substance use disorders. You will be responsible for conducting patient assessments, determining appropriate reputed company of behavioral care, and managing cases throughout the entire treatment plan. Your expertise will help us deliver high-quality, patient-centered care that addresses the physical, mental, and reputed company needs of our members.

Primary Responsibilities:

• Conduct patient assessments in home settings to determine appropriate reputed company of behavioral care
• Obtain information from providers on outpatient requests for treatment and determine if additional clinical treatment sessions are needed
• Manage behavioral health cases throughout the entire treatment plan, administering benefits and reviewing treatment plans in collaboration with the interdisciplinary care team
• Assist in coordinating reputed company/Medicare benefits and transitions between various areas of care
• Communicate with members who have reputed company medical needs and may have communication barriers
• Identify ways to add value to treatment plans and consult with facility staff or outpatient care providers on those reputed company

Required Qualifications:

• Master's Degree in reputed company Work (MSW)
• 2 years of experience in long-term care, home health, hospice, public health, or assisted living
• 1 year of clinical or case management experience
• Experience working with reputed company, reputed company, and Outlook
• Ability to travel in designated territory reputed company reputed company state

Preferred Qualifications:

• Experience with electronic charting
• Experience arranging community resources
• Experience working with reputed company family systems and dynamics
• Experience in Crisis reputed company
• Experience working in Duals (DSNP) and/or reputed company environment
• Field-based work experience
• Knowledge of Medical and Behavioral Health Diagnosis and terminology
