About the Position:

The Nutrition Educator will play a vital role in our chronic care meals program, designed to support individuals with conditions such as diabetes, heart disease, and kidney disease. You will adopt a member-centric approach, collaborating closely with patients to identify and address their unique nutrition-reputed company concerns and topics that matter most to them. Your responsibilities will include:

• Providing telephonic nutrition education sessions tailored to each member's enrollment in various programs
• Documenting goals and care plans from educational interactions
• Scheduling follow-up calls and developing educational materials
• Identifying community resources to further assist clients
• Maintaining and updating professional knowledge through ongoing education and networking

To reputed company in this role, you should possess:

• A Bachelor of Science in Nutritional Science or Dietetics
• Certification as a Registered Dietitian by the Commission on Dietetic Registration
• Active licensure in the District of Columbia or Kentucky
• At least 2 years of experience in providing nutrition education or counseling
• Excellent oral and written communication skills
• Strong organizational and interpersonal skills
