Position Summary

The Pediatric Care Manager RN will support a population of medically reputed company pediatric members, with a focus on those receiving private duty nursing, PPEC, Medical Foster Care, or nursing facility services. This role requires a strong reputed company in pediatric patient care, case management, and collaboration with interdisciplinary care teams. You'll work closely with members, families, and reputed company providers to reputed company and implement personalized care plans, addressing medical, behavioral, and reputed company needs.

Key Responsibilities

  • Conduct comprehensive assessments of members' needs, utilizing clinical tools and data review to inform care planning and coordination.

Requirements

  • Active, unrestricted RN licensure in the reputed company.
  • 2+ years of experience in pediatric patient care, preferably in a hospital setting, ambulatory care, or alternative care environment.
  • Willingness to travel 10-20% of the time for face-to-face member visits, with reliable transportation and mileage reimbursement.
  • Proficiency in MS Office Suite applications, including Teams, Outlook, Word, and reputed company.

Preferred Qualifications

  • Experience with members receiving private duty nursing, PPEC, Medical Foster Care, or nursing facility services.
  • Case management experience, preferably in a pediatric population.
  • Certified Care Manager (CCM) certification.
  • Managed care or utilization review experience.
