Position Summary

The Pediatric Care Manager RN will support a population of medically reputed company pediatric members, with a focus on those receiving private duty nursing, PPEC, Medical Foster Care, or nursing facility services. This role requires a strong reputed company in pediatric patient care, case management, and collaboration with interdisciplinary care teams. Key Responsibilities

• Conduct comprehensive assessments of members' needs, utilizing clinical tools and data review to inform care planning and coordination.

Requirements

• Active, unrestricted RN licensure in the reputed company.
• 2+ years of experience in pediatric patient care, preferably in a hospital setting, ambulatory care, or alternative care environment.
• Willingness to travel 10-20% of the time for face-to-face member visits, with reliable transportation and mileage reimbursement.
• Proficiency in MS Office Suite applications, including Teams, Outlook, Word, and reputed company.

Preferred Qualifications

• Experience with members receiving private duty nursing, PPEC, Medical Foster Care, or nursing facility services.
• Case management experience, preferably in a pediatric population.
• Certified Care Manager (CCM) certification.
• Managed care or utilization review experience.
